Strategies for collecting qualitative and quantitative data to evaluate the effectiveness of the instructional experience will be a set of questions in a workshop evaluation form. This form has been prepared in a Word document as well as a PDF, and can be printed and handed out to participant. If an electronic survey/evaluation is preferred, the information on the document could very well easily be prepared into a service like Survey Monkey.

The quantitative data that will be formally gathered will be included on the workshop evaluation form. This feedback will be analyzed as to the perceived effectiveness and value of the workshop including the facilitator. This information will be gathered by a 5 point Likert scale where 1 is strongly agree, and 5 is strongly disagree.

I will also add a question that involves rating 5 areas from Excellent, Very Good, Good, Fair and Poor.

- What did you most appreciate/enjoy/think was best about the course? Any suggestions for improvement?
- I would be able to do my work better if I knew more about…..
- Please describe the top two topics you would like to learn more about in the next 12 months and what level would interest you (beginner, intermediate, advanced).
- What is your preferred format of learning: Seminar/workshop and how long; Self-study materials; Webinar based learning; and other?
